This self assessment is to be used to evaluate a company's or a functional area's level of maturity and/or capability in the core areas of supplier relationship management (SRM). The five areas evaluated are:

Supplier Management Readiness
Supplier Segmentation
Demand Management
Market Management
Performance Management

The scale used for all factors is a numeric scale of one (1) to five (5). A rating of "1" would indicate the lowest level of maturity or capability for that factor, while a rating of "5" would indicate the highest.
